The 5-Way Packet
How does it Work?
Each 5-Way Packet card has 27 different donation options, ranging from $10 - $100. All you have to do is fill out your contact information on the left side and then what forms of payment you take on the right. You can then send this form to friends and family asking them for donations for your specified cause. All they have to do is select the amount they would like to donate and then provide you with a donation in the form of payment you have chosen to use.
What do I use it for?
You can use the 5-Way Packet to raise money for SCHOOL RELATED activities and trips. Such as athletic fees, uniforms, trips, sports gear, school supplies, and anything else you may need for school. This form was designed for the individual student. We look forward to this packet making a large impact in our school!
How can I get started?
Starting on January 7th, 2023 you can fill in & save the digital copy posted below. Start your fundraiser right off your mobile device. You can use it in the described ways above.
